The equalizer came in the 65th minute when Manchester City defender Josko Gvardiol tapped in a cross from Kristijan Jakic at the back post.
Portugal goalkeeper Jose Sa denied Kramaric's shot as Croatia pressed for a winner, but the point was ultimately sufficient to send them into the quarter-finals.
Scotland secured third place in the group with a 2-1 victory in Poland, thanks to Andy Robertson's header in injury time.
